From Burnout to Breakthrough: How I Built a Scalable Remote PT Career with Remote Therapeutic Monitoring

If you had told me in 2018 that I’d be running a nationwide Remote Therapeutic Monitoring consulting business from an RV, I would have laughed, sipped my coffee, and gone back to charting in my tiny therapy office. Back then, I was like many physical therapists — working long hours in a busy outpatient clinic, juggling productivity pressures, and wondering if I’d ever find a way to have more freedom without leaving the profession I loved.
Fast forward to today: I’ve traveled to all 48 contiguous states, explored 3 Canadian provinces, homeschooled my kids between patient calls, and helped hundreds of clinics successfully implement RTM. It’s been a wild ride, and it all started because I was burnt out and looking for something scalable, sustainable, and still connected to patient care.
Burnout, Family Priorities, and a Big Leap
By 2019, my wife (also a PT) and I were both feeling the strain. We loved servicing patients through our profession, but the profession had lost it's luster and began to feel draining. Our two kids were at that impressionable, curious age - able to explore and play, yet not yet tied down with activity obligations - and we knew these years were too valuable to spend buried in clinic schedules.
So, we sold it all. House, furniture, the white water raft (that hurt the most) — gone. We bought an RV and hit the road as travel PTs, swapping between working parent and homeschooling parent. Our mission? Follow the sun, see incredible places, and reclaim some balance. It wasn’t always easy (there’s a learning curve to repairing your own RV plumbing at 10 p.m.), but it was worth it.

The RTM Discovery
About 2.5 years into our adventure, in February 2022, I accepted a temporary contract with my former employer back from where we started this crazy adventure. While there, I heard about something new: Remote Therapeutic Monitoring. New CPT codes, new possibilities — but also, no clear roadmap. Most clinics were ignoring it because it was ‘too new’ and ‘too complicated.’ That’s when my former boss, and friend, made me an offer: figure it out and run it remotely.
Challenge accepted. I dove into every regulation, payer policy, and documentation requirement I could find. I made mistakes (a lot of them), learned what didn’t work, and eventually built workflows that actually delivered patient engagement, clinical value, and consistent revenue.
The Hard-Won Lessons of RTM
Here’s what I learned the hard way — and what I now teach in my courses so you don’t have to repeat my mistakes:
• RTM is a service, not just a billing code. If therapists and patients don’t see the value, it won’t work.
• The right platform matters, but the right workflows matter more.
• Someone in the clinic must ‘own’ RTM — a champion who keeps it moving, tracks billing, and ensures compliance.
• Engagement is everything. Without consistent patient interaction, you’re just collecting empty data.
These lessons came from trial, error, and a lot of late-night troubleshooting from various campgrounds.

Once my first RTM program was running smoothly, word started to spread. I began helping other clinics implement RTM, each with their own unique patient populations, therapist personalities, and administrative quirks. Today, I’ve worked with hundreds of clinics across the U.S., refining my workflows with each new project.
The best part? I get to show therapists and clinic owners that RTM can be a game-changer — creating new revenue streams, improving patient care, and offering more flexibility in how and where therapists work.
